In accordance with the most recent (Current as of January 4 2022) Government of Ontario Covid-19 regulations, we require these screening questions to be filled out and submitted by the lead guest (who made the booking):

Cough, sore throat, or trouble swallowing
Runny/stuffy nose or nasal congestion
Decrease or loss of smell of taste
Nausea, vomiting, diarrhea, abdominal pain
Not feeling well, extreme tiredness, sore muscles

(required) All members of our group have not travelled outside of Canada in the last 14 days.

All members of our group have not had close contact with a confirmed or probable case of Covid-19
